I put my phone and keys on my life vest, modified and added additional zipper pockets for quick access:

For my cell, I have a small iphone 3, I use one of those cheap water proof pouch, wrap the phone in double zip lock bags with dessicant to absorb water vapors. Water sometimes get through the pouch, but the zip locks stay clear all the time. I don't really need to access my phone while riding. All I care if I have to self rescue is for me to get to shore. So I just relax, take my time, and follow the least resistant flow to get back on land.
For open water. I have really small EPIRB and radio, so I just hang them on each side of the vest, zippered in on the custom pouches I made, easily accessible, so no biggie.
The trick is to modify your vest/tool carrier so your gear stay put if you get into a wash where you tumble around.
For anything that I customize concerning safety, I make sure to test them how strong the equipment is going to be.
